Findings: Alleged Victim (AV) relies on the facility for h/h care. AV had a known history of unsafe behaviors, including being in danger of hurting him/herself or someone else with weapons. On or about May 24, 2021, four days after moving into the facility, AV was found with knives packed in his/her belongings. The facility failed to ensure a safe environment, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
