Findings: On or about May 21, 2021, Witness #1 (W1) pushed his/her walker into the back of the Alleged Victim's (AV) legs three times, causing pain and bruises. W1 is known to cause altercations and is care planned to be assisted and redirected to avoid altercations. W1 was not redirected as care planned, which placed AV at risk for harm. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03158 $188.00 fine assessed
