Findings: On or about May 12, 2021, the facility ran out of the Alleged Victim's (AV) incontinence supplies. The facility provided a different type of incontinent supply to AV, which was too small and caused an allergic reaction to AV, causing unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
