Findings: On or about late December 2020, it was discovered that the Alleged Victim (AV) had multiple unauthorized transactions completed from October 2020 through December 2020 using his/her debit card. All of the charges were reimbursed to AV, however, the charges were made by an unknown individual, and this person is responsible for theft, which is a violation of resident rights, is considered neglect of care and constitutes financial exploitation. The facility failed to protect AV from financial exploitation, which is a violation of Oregon Administrative Rules.
Sanction: ALFCP21-02994 $250.00 fine assessed
