Findings: Alleged Victim (AV) relies on the facility for a safe environment. Witness #1 (W1) has history of inappropriate behaviors towards staff and other residents. On or about May 21, 2021, W1 was found in AV’s room, placing AV at risk for harm. The facility’s failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
