Findings: The facility failed to provide basic services necessary to maintain the health and safety of Alleged Victim (AV) which resulted in AV sustaining several falls in the month of May 2021. An investigation determined that the facility failed to follow AV's service plan to have 1 on 1 staff observations during waking hours which resulted in harm to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02971 $1500.00 fine assessed
