- On or about May 24, 2021, the Alleged Victim (AV) and Witness #1 (W1) engaged in an altercation where W1 bit AV. There were no injuries, however, AV was placed at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
