Findings: Alleged Victim (AV) has a known history of falls. Between February 5, through May 8, 2021, AV had seven (7) falls/or had been found on the floor. On or about May 19, 2021, AV was displaying agitation and anxious behaviors. AV was placed in the family room and was left alone. AV was later found on the floor of the family room and was sent to the hospital. AV was diagnosed with a urinary tract infection and a subdural hemorrhage. The facility failed to properly plan care and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-03091 $1500.00 fine assessed
