- The facility failed to ensure supervision and staff support regarding known behaviors related to the Alleged Victim (AV) and Witness #2 (W2). An incident occurred between the two where W2 hit AV in the face. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
