Findings: The Alleged Victim (AV) was care planned to receive showers two times per week. On multiple occasions, AV did not receive two showers a week and at times did not receive one shower. Due to this, AV had skin rashes develop, causing unnecessary discomfort. The facility was short staff and unable to meet the scheduled needs of residents. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
