Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about May 30, 2021, staff checked on AV and at that time AV appeared to be sleeping in his/her bed. Approximately 15 minutes later, AV’s call light was activated. Staff responded to the call light 10.83 minutes after AV’s call light was activated. There was a prior incident in which AV’s call light was not responded to until 13.80 minutes later. The facility failed to answer call light in a timely manner, which is a violation of resident rights, is neglect of care and constitutes abuse.
