Findings: Alleged Victim (AV) is unable to use his/her call light and is often confused. Alleged Perpetrator 2 (AP2) has wrapped AV’s call cord around AV to go off is AV moved. AP2 was provided with appropriate training and signed of on his/her training checklist acknowledging he/she understand supportive device and possible restraint qualities, as well as mobility/safe resident handling. AP2 failed to use restraint properly, which is neglect of care and constitutes abuse. The facility failed to assure restraint was properly use, which is a violation of Oregon Administrative Rules.
