Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about June 3, 2021, the facility was made aware that AV had an injury/skin tear of unknown cause to the right arm. AV’s family made multiple calls to the facility regarding this event, but the calls were never returned. Between June 3rd, through August 20th, 2021, the facility made no demonstration that the incident was investigated. The facility failed to investigate injury of unknown origin to rule out abuse, which is a violation of resident rights, is neglect of care and constitutes abuse.
