- Findings
- Alleged Victim has a history of falls, including falls while tripping on furniture. On or about May 26, 2021, AV was walking to the activity room and tripped and fell over a shampooer that was left on the floor resulting in multiple injuries. The facility failed to provide a safe environment which is a violation of resident rights, is neglect of care as defined in OAR 411-020-0002(1)(b)(A)(i) which constitutes abuse.
