Findings: The facility failed to ensure supervision and staff support regarding known fall risks related to the Alleged Victim (AV). Av had multiple interventions in place for falls, however, they were not effective. On or about June 6, 2021, AV suffered an injury fall and was sent to the hospital and diagnosed with a subdural hematoma, bruises and a laceration to the head. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03031 $375.00 fine assessed
