Findings: The facility failed to follow Alleged Victim's (AV) and Witness 2's (W2) care plan which resulted in potential harm to AV. On or about March 19, 2021, AV and W2 had a resident-to-resident altercation in which both ended up falling on the floor in the hallway by AV's room. The facility's failure is a violation of resident rights is considered neglect of care and constitutes abuse.
