Findings: The facility failed to provide a safe environment for Alleged Victim (AV) which caused AV emotional harm. An investigation determined that AV was uncomfortable around Alleged Perpetrator 2 (AP2) and on or about June 10, 2021, the facility agreed to a safety plan wherein AP2 would not provide care on the floor where AV's apartment was located. AP2 continued to work on the floor where AV's apartment was located after the safety plan was put into place. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03232 $250.00 fine assessed
