Findings: Alleged Victim (AV) relies on the facility for his/her care. AV received all but two showers in the last two months of residency, there are no refusals noted for the showers missed. There were discrepancies in the care of AV’s nails, one part of AV’s care plan states that AV is independent in nail care, another part of the care plan states that staff are to trim and observe nails. AV received nail care only twice in the last two months of residency with one refusal noted. AV’s toenails were curling into AV’s toes. The facility failed to provide service, which is a violation of resident rights, is neglect of care and constitutes abuse.
