Findings: The facility failed to follow the Alleged Victim's (AV) care plan to assist with showers three times per week. The facility failed to have a process to track showers for AV and due to AV's behaviors, two staff are needed to shower AV. The facility didn't have enough staff to ensure AV's showers were given as care planned. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
