Findings: Alleged Victim (AV) has a known history of exit seeking or elopement behaviors. On or about June 10, 2021, AV went out through the front door of the facility behind another person and was outside for approximately two to three minutes before AV was discovered by staff. The facility failed to provide a safe environment to AV, which is a violation of resident rights is neglect of care and constitutes abuse.
