Findings: Alleged Victim (AV) relies on the facility for his/her care. Between June 8th through July 3rd, 2021, Witness 1 (W1) experienced six (6) episodes of sexual behaviors towards AV. On or about July 13, 2021, despite instructions to staff to keep AV and W1 separated, W1 had AV on his/her bed. W1 had aggressive sexualized behaviors, including attempting to pin AV down. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of sexual exposure by W1, which is a violation of resident rights, is neglect of care and constitutes abuse.
