Findings: The Alleged Victim (AV) is care planned to be checked 5 times per night to be taken to the bathroom to prevent falls during the night hours. Facility staff admit they check on AV during the night hours, however, do not wake AV and take him/her to the bathroom as care planned. On or about June 13, 2021, AV was found in the morning with a bruise and dried blood on his/her face. AV was sent to the hospital and diagnosed with an orbital fracture. the facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
