Findings: Alleged Victim (AV) has a known history of falls. On or about June 16, 2021, AV had an unwitnessed fall and was found on the floor. AV was found lying on the floor on their left side on a pool of blood on the back of AV’s head. AV was sent to the hospital due to a head laceration and staples were used to close the laceration. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
