Findings: On or about May 30, 2021, ti was noticed that the Alleged Victim (AV) had a bruise on his/her left arm and an injury on his/her wrist. The facility failed to document or complete and investigation into an injury of unknown cause for these injuries. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
