Findings: Alleged Victim (AV) is care planned to receive nail care and is a two-person, or more, full assist with transfers. On or about June 17, 2021, it was reported by Witness 2 (W2), that AV had not been receiving nail care for one to two years. During the investigation, it was also reported that on or about June 14, 2021, AV could not be transferred out of bed because of staffing shortages. The facility failed to provide nail care which resulted in AV experiencing dry skin and overgrown toenails. The facility failed to assist with transfers which resulted in AV not being transferred out of bed. These failures are a violation of resident rights, is neglect of care which constitutes abuse.
