Findings: The facility failed to protect Witness 1 (W1) from a resident-to-resident altercation in which Alleged Victim (AV) slapped W1 five or six times while W1 was sitting at a dining room table. AV has a history of aggressive behaviors and has had several resident-to-resident altercations. The facility's failure to provide a safe environment for W1 is a violation of resident rights, is considered neglect of care and constitutes abuse.
