Findings: Alleged Victim (AV) relies on the facility for a safe environment. On or about June 04, 2021 and June 12, 2021 AV had sexual encounters with different residents. It was not until approximately June 24, 2021 a Safety plan for AV was developed. The facility failed to provide a safe environment and properly care plan which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03291 $1500.00 fine assessed
