Findings: The facility failed to provide a safe environment for Alleged Victim (AV) by failing to provide medical care in a timely manner after AV fell on or about June 9, 2021. An investigation determined that AV was not sent to the hospital for treatment until June 22, 2021, in which AV was diagnosed with fractured femur/knee. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
