Findings: Alleged Victim (AV) relies on the facility for his/her care and to manage his/her medications. According to witnesses, on or about June 18, 2021, when Alleged Perpetrator 2 (AP2) returned from lunch, AP2 was under the influence and passed medication to AV. AV was exposed to potential harm. AP2's actions are considered neglect of care and constitute abuse. The facility failed provide a safe environment which is a violation of Oregon Administrative Rules.
