Findings: Alleged Victim (AV) relies on the facility for his/her care. AV's care plan states facility is to have eyes on AV when AV is up and awake to ensure AV does not wander into other’s rooms. On or about June 27, 2021, AV was looking for a personal item and entered Witness #1 (W1) room. AV and W1 engaged in an altercation, resulting in W1 pushing AV. AV hit his/her head and c/o pain in his/her shoulder. The facility failed to follow the care plan, placing AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00250 $500.00 fine assessed
