Findings: Alleged Victim (AV) relies on the facility to manage his/her medications. On or about June 26, 2021, Alleged Perpetrator 2 (AP2) administered AV a medication not prescribed to AV. AV asked AP2 what the medication was prior to taking the medication as AV did not recognize the medication. AP2 stated they did not know. AV experienced side effects due to the medication error. The facility failed to ensure that AP2 was properly trained to pass medications. The facility and AP2 failed to provide a safe medication administration system, which is a violation of resident rights, is neglect of care and constitutes abuse.
