Findings: The facility failed to provide service to the Alleged Victim (AV). On or about April 28, 2021, AV was found with white patches in his/her mouth. AV was taken to urgent care where he/she was found to have thrush from not having his/her teeth brushed and cleaned daily and prescribed an antibiotic. AV returned to the facility and later he/she was found to be crying and not eating or drinking and was taken to the Emergency Room for evaluation. At the hospital, it was discovered that AV was dehydrated and had a urinary tract infection. AV was admitted to the hospital where he/she received IV fluids and IV antibiotics for seven days. The facility’s failure to provide necessary services to AV placed AV at risk for serious harm. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
