Findings: Alleged Victim (AV) relies on the facility for his/her care. AV has a history of falls including falls that resulted in injury. AV is documented to have experienced five falls in less than ninety days. AV has sustained two cuts on his/her head, one requiring stitches, and multiple bumps and bruises. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
