Findings: On or about March 17, 2021, Alleged Victim (AV), Witness 1 (W1), and Witness 2 (W2) were involved in a resident-to-resident altercation where W1 hit AV and AV hit W2 while in the dining room area. An investigation determined that the facility failed to follow AV's care plan which placed W1 and W2 at risk for potential harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
