Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about June 29, 2021, AV was in a wheelchair self-propelling with his/her feet from a hallway into the dining area. AV propelled him/herself to the first table and chairs in the dining area and he/she used a chair to assist his/her stand. AV had a tab alarm that stretched and visibly disconnected from the wheelchair. AV then stood behind a chair and used it as well as the table to walk to the side of the table. AV then turns him/herself sideways and placed his/her upper left leg on the top of the table and his/her left hand down towards the center of the table. AV then got to the table which caused the table to tilt under AV’s added weight and causes AV to fall and land on his/her hip. The fall cause AV to sustain a hip fracture. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
