Findings: Alleged Victim (AV) relies on the facility for his/her care and a safe environment. AV and Witness #1 (W1) have a history of verbal and physical altercations. On or about June 29, 2021 AV and W1 had a verbal and physical altercation resulting in AV having a scratch on AV’s cheek. The facility failed to care plan interventions for AV to protect AV from resident to resident altercations with W1, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
