Findings: The facility failed to provide basic care, services, and/or safety of Alleged Victim (AV) which resulted in AV suffering falls on February 19, 2021, March 29, 2021, May 29, 2021, and June 30, 2021. On or about June 30, 2021, AV fell, was sent to the hospital for treatment, and was diagnosed with a broken right hip. An investigation determined that facility failed to implement reasonable interventions to address AV's ongoing falls and physical decline which is a violation of resident rights, is considered neglect of care and constitutes abuse.
