Findings: Witness #1 (W1) had been exhibiting aggressive behaviors and had an altercation with a different resident earlier in the day. On or about July 1, 2021, W1 walked by the Alleged Victim (AV) and kicked him/her in the knee. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
