Findings: The Alleged Victim (AV) was a known exit seeker and the days leading up to this incident, had been exhibiting increased behaviors. On or about July 4, 2021, the AV eloped from the facility and fell, sustaining an abrasion to his/her forehead and a black eye. AV was sent to the hospital for evaluation and returned the same day. It is suspected that AV exited the building while following a family member. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
