Findings: On or about July 3, 2021, late in the evening, the Alleged Victim's (AV) call light was on, staff responded. When staff opened AV's locked door, Alleged Perpetrator #2 (AP2) was seen by staff, jumping up from AV's bed, and holding on to his/her pants. AV was lying on his/her bed, naked from the waist down. When asked, AV stated that he/she believed AP2 was trying to have sex with him/her. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes sexual abuse. The facility allowed AP2 to work in the facility, unsupervised, without an approved background check. AP2 falsified information on his/her background check documentation and it was later discovered that AP2 is a felon. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
