Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) and Witness #1 (W1) regarding their risk of being involved in altercations. On or about July 2, 2021, AV and W1 were found pushing and punching each other. The facilities failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
