Findings: The Alleged Victim (AV) is a known fall risk and has had 5 falls in the month of June. On or about July 6, 2021, AV fell and his his/her head, resulting in a laceration to his/her head and ears. AV was not sent out to the hospital due to family refusal. Despite AV's falls in the recent past, the facility failed to implement interventions to reduce AV's risk of falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
