Findings: On or about July 2, 2021, the Alleged Victim (AV) was seen placing hand sanitizer in his/her coffee. AV's interim service plan stated that AV required hands on monitoring in common areas and hand sanitizer shouldn't be within AV's reach. Between July 2 and July 7, 2021, hand sanitizer was found in AV's room and on a dining room table within AV's reach, placing AV at risk for harm. The faciilty failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03131 $250.00 fine assessed
