Findings: Alleged Victim (AV) relies on the facility for h/h care. AV has a history of wandering into other resident’s rooms and taking items that do not belong to h/h. On or about July 8, 2021, AV entered a resident’s room where a resident-to-resident incident occurred, and AV had to be sent to the hospital for head injuries. The facility failed to appropriately care plan, which is a violation of resident’s rights, is neglect of care.
