Findings: On or about July 7, 2021, Alleged Victim (AV) suffered an unwitnessed fall with injury. AV approached Alleged Perpetrator 2 (AP2) after the fall and AP2 provided minimal assistance to help AV with his/her injury when AP2 only offered AV paper towels and no other assistance. An investigation determined that AP2 did not follow the facility's protocol for reporting falls which resulted in AV suffering unnecessary pain and discomfort. AP2's actions is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules. The allegation that Alleged Perpetrator 3 (AP3) neglected AV was investigated and determined there was no wrongdoing by AP3.
