Findings: Alleged Victim (AV) relies on the facility for his/her care. From February 2021, through July 2021, AV had had approximately seven (7) incidents of injuries of unknown origin. For the year 2021 to the date of allegation, AV had approximately 16 falls. On or about July 9, 2021, AV was found with a bruise of unknown origin on his/her right elbow. The facility failed to care plan appropriately and implement interventions to mitigate AV's risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
