Findings: The facility failed to provide a safe medication administration system for Alleged Victim (AV) by failing to administer AV's thyroid medication as ordered which caused AV to have significant weight gain. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse. Alleged Perpetrator 2 (AP2) neglected AV by failing to provide basic care and/or services necessary to maintain the health and safety of AV, which resulted in unreasonable discomfort and a serious loss of personal dignity. AP2 failed to follow AV's care plan to provide timely and appropriate toileting assistance which caused AV unreasonable discomfort and loss of personal dignity. AP2's actions is considered neglect of care which constitutes abuse.
