Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about July 8, 2021, AV, and Witness 1 (W1) had a physical altercation with each other. AV suffered a facial scratch from the physical altercation. On or about July 23, 2021, AV was sitting in the facility main room and W1 approached AV and hit him/her in the eye. AV suffered a cut above the eye and a black eye. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
