Findings: The Alleged Victim (AV) is care planned not to be left alone is his/her wheelchair. On or about July 9, 2021, AV was left alone in his/her wheelchair in the common area, fell from the wheelchair and sustained bruising and a cut requiring transport to the emergency room where he/she received steri-strips to close the wound. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
