- Findings
- Alleged Victim (AV) relies on the facility for his/her care. Witness 1 (W1) has a history of angry behaviors. On or about July 12, 2021, W1 made threating statements to harm AV. The facility failed to appropriately care plan and implement reasonable interventions to address W1s ongoing behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
